Although most of our patients like some sort of light sedation, we can do breast augmentation, liposuction, and eyelid procedures with pure local anesthetic. We often will give a relaxation pill prior to this, but local anesthetic does the job, and if you’re a calm and relaxed individual who doesn’t want any intravenous medicine, we’re certainly happy to help you using the pure local anesthetic liposuction technique. A few of our patients choose this method, and we can make you perfectly comfortable with it. It does take a little bit of talking during the procedure and hand-holding, but pain isn’t a significant problem at all; in fact, most of our patients have no pain and are able to go home almost immediately after the procedure is complete. Patient: About 25 or 26 years ago, I had a tummy tuck by another surgeon: it was the one where they cut you and has the scar. I was unhappy with it. I did not like the scar. The scar went from hip to hip plus I developed keloids too from it. I went all these years unhappy and then about 12 years ago, liposuction. became popular and I decided to come see you, Dr. Yoho about it and the first thing you did, which I loved was you put me in a room and I watched the video. I watched the video with the old-fashioned tummy tuck and with the liposuction With the liposuction, there were no scars just a little needle marks and that was it. As soon I saw that, I said this is it and I noticed the difference in the body, which I felt that with the other surgery I am looking more like a square where this surgery it was contoured toward the body more. So I signed up immediately to have you do it for me and the reason I took me so long was because like I said I thought this was for rich people only and I was surprised that the prices and everything else. I had it done on a Wednesday and by Friday I was out with my girlfriend shopping for some new tops because my other tops were too big already. It was amazing. It totally amazed me. Then my girlfriend named ---------, she was so amazed with it that she was probably the largest case you did as I recall it that time and she had the same thing done. We were just flabbergasted with it. Then I came back later and I had these lumps where when I put my bra on I had these lumps behind my bra straps that bothered me. I came and you got rid of those for me so when I put on a dress it was nice and flat, I did not have all these little lumps. Then you did my cheekbones. I did not have this all at ones. This is like 3 to 5 years later. Because my face, I was tired or people asking me if I was tired and I would say no I am not tired. My face was flat. There was no shape to it. So of course, you did my cheekbones and I am crazy about it. I think you did one more thing for me. I think it was around … I know you did 1 more thing. I hope it was my eyes or … Oh yes you did. I think you did the bags under my eyes also. It just makes me feel better. The older I get, I feel better. I have a very loving husband. He loves me the way I am, but it makes me feel good and makes me happy. If it makes me happy, it makes them happy.I trust you. I love the work you do. While this may be a joke for a lot of people, it is no joke for the people who have the problem. These men wear t-shirts and extra layers on their upper body to conceal themselves and won’t go to the beach and make excuses their whole lives. This problem is easily surgically fixed and no one should hesitate because it is not that expensive and not a complicated procedure. Generally, the patients have an excess of the male hormone testosterone when they are entering maturity at 14 to 16 years of age. This excess of the male hormone is altered by the body into female hormone. This is just the way the hormone is removed by the body. When the female hormone appears on the scene, it has the ability to stimulate breast tissue development in the male just like the females. Sometimes even lactation or milk production occurs in small quantities. In any case, this breast tissue tends to persist throughout life. An additional component of the problem is the surrounding fat, which really can almost look like a female breast.

These patients are very shy and hesitant to show us their chests, but we are used to the issue and do our best to put everybody at ease. Our Pasadena/Los Angeles Cosmetic Surgery technique is as follows. First, the patient is given relaxation medicine so they are not aware of what is going on. Second, local anesthetic is inserted underneath the skin. Third, liposuction is performed to remove any surrounding fat. Fourth, we make a half-inch incision in the crease below the breasts in order to go in and remove the breast gland tissue. Since there is lots of local anesthetic, the area usually does not bleed much. Careful attention is required by the surgeon to be sure that everything is removed in an even fashion and also that the muscle underneath the breast tissue is not damaged, because this can cause bleeding. We attempt to make the area not too thick and not too thin in order to produce a normal skin texture and skin fold when the area heals. After this is done, we attempt to stop any bleeding with an electric needle, and the skin is closed very carefully in order to conceal the incision.

This incision may be made through the areolar area, but we found that over the years the areolar area is more susceptible to distortion and scarring then than the skin underneath the breast in the crease between the breast and the abdominal skin. This incision that we like may take 6 to 12 months to fade but generally does better and is less noticeable than an areolar incision. Men like nice nipples too just like women!

A common complication of this surgery is fluid formation in the area where the fat and breast tissue was removed. Not to worry, at 1 to 3 days after surgery, if this happens, we will remove this fluid from the cavity where the breast tissue was taken, and then we replace the elastic wrap. Generally this solves the problem. Often this is done using light relaxation medication and takes just a few minutes. All of these are included in the price of the procedure as well as all your followups, your elastic wraps, and so on and so forth.
NOTE: a few rare conditions may make individuals grow gynecomastia or “man boobs”, and you should speak with your physician about the possibility that you need blood testing for this possibility.

What is the "Brazilian Butt Lift"?

The Brazilian Butt Lift consists of liposuction of the areas around the buttocks, usually including the area above the butt or “sacral” area and the hips and often the outer thighs. Then, sometimes, fat is transplanted into the buttocks to make them fuller. Contrary to what is said elsewhere, the fat should be transplanted into the lower two-thirds of the buttocks, not the upper two-thirds. There are some doctors who are spinning the fat, so as to make it denser. This does not improve the result. The bottom line is that any given butt can take only so much fat in any one operation or the transplanted fat tends to cause problems like infection. Infection is a risk anyway even if everything is done to the best possible standard. If you have a big butt this technique can be used to make it much bigger. If you have a small butt, the Brazilian Butt Lift can only do so much. Dr. Yoho has been performing this procedure for over a decade. Results vary with the individual, with some people getting almost all the fat to “take”, making the butt more shapely. Others, such as smokers, will have very little retention of the fat. If you are a smoker, it’s probably not worth it.

There are surgeons who are putting implants, made of silicone, into the buttocks. These implants generally go under the muscle and in Dr. Yoho’s opinion often cause unnatural results, with somewhat strange bulges on top of the butt. See results at other internet sites. Fat transplant methods can be repeated if the patient wants more fullness.

Note that the above patient didn’t even have fat transplant to the buttock, but the butt was lifted and the shape was improved anyway. This is because, as noted above, the areas around the butt were shaped beautifully. An additional factor is that slight weight gain (actually the patient stabilizing at a similar weight as before) after a relatively large liposuction like this one can produce very nice fullness of the breasts and buttocks, where the remaining fat tissues can grow a little. There may not be any ultimate net weight loss, but the patient sure looks a lot better.

I have one additional note about this sort of case. It’s generally a mistake (of course there are exceptions) to liposuction the buttocks themselves. Liposuction of the area around the buttocks is best. This usually produces the most improved shape.
